This initiative transforms disease surveillance and climate-sensitive outbreak tracking in Ethiopia’s primary healthcare units by transitioning from paper-based reporting to digitized, integrated surveillance systems. Operating in resource-limited settings vulnerable to climate variability, the project equips healthcare workers with digital tools to streamline data collection, reduce reporting lags, and improve overall data completeness. By integrating daily clinical triage and laboratory reports into a unified digital platform, the initiative enables early detection of climate-sensitive infectious diseases, bridges gaps between rural clinics and regional health bureaus, and strengthens healthcare infrastructure resilience against climate-induced disease spikes.

Since implementation, the initiative has successfully digitized surveillance workflows across targeted primary healthcare facilities, training over 90% of frontline health workers, triage nurses, and lab technicians in system usage. Timeliness and completeness of weekly surveillance data improved dramatically toward a 95–100% threshold, while outbreak detection times dropped significantly. By fostering data-driven decision-making, the project has enhanced local capacity to monitor climate-sensitive health threats, strengthened institutional IPC compliance, and created a scalable model for climate-resilient digital health systems across regional health networks.
